Post Adoption Search Form

Effective July 1, 2024 adopted individuals born in Minnesota 18 or older are able to request a non-certified copy of their original birth records from the Minnesota Department of Health.

When you submit a search inquiry through Foster Adopt Minnesota, please expect a response within about 30 days. Sometimes, this period may be extended as we research additional resources. If you know your placing agency that facilitated your adoption, please contact them directly for post adoption services.

If you are a child of an adoptee and the adopted individual is deceased, you will be asked to provide the death certificate and your birth certificate.
